7 Companies Building Patient Intake Software for Specialty Practices

7 Companies Building Patient Intake Software for Specialty Practices

Specialty practices don’t struggle with intake because forms are hard. They struggle because a physical therapy plan runs into visit limits, imaging needs prior authorization, and behavioral health coverage is often carved out to a different payer.

A clean form doesn’t answer any of that. This list compares seven companies building patient intake software for specialty practices, grouped by the intake step each one actually handles.

TL;DR

  • There is no single best intake tool. The right pick depends on where intake breaks: paper and check-in, patient phone calls, faxed referrals, or payer follow-up after the eligibility check.
  • In Experian Health’s State of Claims 2025 survey, 32% of providers blamed registration data for denials and 35% blamed authorizations. Both problems start at intake.
  • The 2024 CAQH Index puts a manual eligibility and benefits check at $8.57 for a provider versus $2.00 electronically.
  • Self-funded plans often return partial benefits on a 270/271 check, so someone still calls the payer or logs into a portal.
  • CMS-0057-F requires prior authorization APIs from Medicare Advantage, Medicaid, CHIP, and federal exchange plans starting January 2027, but commercial self-funded plans fall outside the rule.
  • Judge intake software by completed cases and the exact fields it writes back to your EHR, not by submitted forms.

How We Evaluated the Companies?

Every company was assessed against the same five criteria:

  1. Specialty fit. Does the vendor name specialty practices it serves and handle their rules, such as visit limits or carve-outs?
  2. Intake scope. Forms only, or also insurance capture, eligibility, benefits, authorization, and patient calls?
  3. EHR and PM write-back. Does data land as structured fields in your systems, or as a PDF someone re-keys?
  4. Named security. HITRUST, SOC 2, HIPAA, and BAAs. Named beats “secure.”
  5. Published proof. Outcomes from real practices, with vendor-reported numbers labeled as such.

The list does not measure star ratings or company size. Order reflects fit for specific situations, not overall quality.

1. MindK

Best for: specialty networks where staff still finish eligibility and benefits verification by phone or portal.

The MindK healthcare team builds intake and payer agents for the cases simple automation gives up on: self-funded plans, long IVR menus, and portals that change without notice.

Voice and portal agents stay on a case until they have an answer, then write network status, coverage, auth requirements, copay, deductible, and out-of-pocket max back to your system of record as structured fields. Failed calls are sorted into 13 outcome classes, so staff know exactly what to fix.

Integrations include Epic, Oracle Health, athenahealth, eClinicalWorks, MEDITECH, and NextGen over X12 270/271 and 278, FHIR, and HL7, and PHI is anonymized before any third-party model sees it. MindK’s own GoodBilling product runs this intake flow for 300 practices.

Potential limitation: not self-serve, with no ready-made patient forms or kiosk.

Our take: the strongest fit when the form is fine but the payer work behind it isn’t.

2. Assort Health

Best for: specialty practices where intake still happens over the phone and new patients give up on hold.

Assort Health’s AI phone agents pick up when the front desk can’t. Callers can book visits, complete intake and triage questions, check referrals, request refills, and make payments, including after hours. The company names 23+ specialties and claims 15+ EHR and PM integrations, with athenahealth the strongest.

Peninsula Orthopaedic reports 75% lower call abandonment, and Barrington Orthopedic reports 34% fewer unanswered calls (vendor-reported).

Potential limitation: patient-facing only, so benefits and authorization lookups with payers stay with your team.

Our take: start here if patients never reach the form in the first place.

3. Phreesia

Best for: practices of any size replacing clipboards and slow check-in.

Phreesia lets patients check in on their own: they update registration, sign consent forms, answer questionnaires, and pay copays before or at the visit, with VoiceAI covering the phone side. More than 4,700 organizations use it, go-live can take as little as six weeks, and it natively supports 16+ PM and EHR systems.

Security credentials include HITRUST CSF, SOC 2 Type 2, and PCI Level 1. Phreesia reports a 73% average increase in time-of-service collections within six months (vendor-reported).

Potential limitation: benefits that the electronic eligibility check doesn’t return still need a person.

Our take: the safe, mature choice for digital check-in, and it can run alongside payer automation.

4. Yosi Health

Best for: specialty practices where history-taking slows visits as much as paperwork does.

Yosi Health sends patients electronic intake forms before the appointment, and their answers land in the EMR in real time.

Beyond consent and screening forms, it offers problem-specific HPI templates, so clinicians start the visit with the history already filled in.

Testimonials come from ENT, endocrinology, eye care, and pain management practices, and the company references Elation and the athena Marketplace for integrations.

Potential limitation: no AI features are described, so treat it as a forms and check-in tool rather than an agent.

Our take: a good fit when clinical intake quality matters as much as front-desk speed.

5. Nanonets Health

Best for: specialty practices whose real intake queue is the fax inbox.

Nanonets Health grew out of document AI. Its agents read referral packets and insurance cards across 20+ document types, then carry the case into eligibility, prior authorization, and scheduling without re-keying.

Named specialties include physical therapy, behavioral health, dental, dermatology, pain management, radiology, and sleep medicine.

Integrations cover athenahealth, NextGen, eClinicalWorks, MEDITECH, MEDHOST, OfficeAlly, and CollaborateMD, and the company cites HIPAA and SOC 2. One customer reports days in A/R falling from 54 to 42 (vendor-reported).

Potential limitation: published outcomes are billing metrics, so ask for referral-to-scheduled time.

Our take: test it on a week of real referral packets before deciding.

6. SPRY

Best for: PT, OT, and speech therapy groups willing to run intake, notes, and billing on one system.

SPRY collects demographics, insurance, consent, history, and outcome measures before the visit and offers kiosk check-in.

Its standout feature for rehab is showing remaining visits, authorization status, and copays before the patient checks in, so staff can request a new authorization before covered visits run out.

The vendor reports more than 1,000 rehab clinics as customers, and BEST Physical Therapy reports 95% fewer authorization denials (vendor-reported).

Potential limitation: it runs on its own EMR and billing, so groups keeping their current EMR or adding other specialties would need to migrate.

Our take: a tight fit for rehab-only groups ready to consolidate.

7. IntakeQ

Best for: small therapy and behavioral health practices that need HIPAA-compliant forms fast.

IntakeQ provides online registration, history, and questionnaire forms with conditional logic, e-signatures, and reminders.

It serves counseling, psychiatry, psychology, physical therapy, occupational therapy, and lactation practices, connects with Elation and RevolutionEHR, and offers Zapier and an API for other systems. More than 10,000 practitioners use it, and pricing is published on its website.

Potential limitation: no insurance features are described, and it isn’t built for multi-location networks.

Our take: enough when most patients are self-pay or on simple commercial plans.

How to Choose and How This List Was Built?

Each company was assessed on the five criteria above using its public product pages, case studies, and published integration and security information.

Metrics marked as vendor-reported come from the companies themselves. If a capability isn’t mentioned, it wasn’t publicly claimed, which doesn’t mean it’s missing.

Before you buy, run these checks against your own practice:

  • Count payer contacts. Pull last month’s intake cases and count how many needed a portal visit or a call. A handful means forms are enough. Hundreds means payer follow-up is the real problem.
  • Check write-back. Ask for the exact fields written to each EHR and PM system, plus a sample record.
  • Ask about exceptions. When the tool can’t finish a case, staff should get it back with a reason and a next step, not an empty field.
  • Price per completed case. Include staff time on exceptions and the upkeep of every integration.
